Please submit this questionnaire after you have registered with Road Scholar. This information will help us to make your Universe experience as comfortable and beneficial as possible.

Please note that lodging is in shared apartments on the UC Santa Cruz campus. The apartments have shared kitchens, living rooms, and bathrooms. Single bedrooms are available, but private bathrooms are not. Ground floor and fully accessible ADA apartments are available but limited. Please note if you would prefer this option.
